A private medical invoice with procedure codes, dates of service and insurer details.
The Medical Invoice Generator carries what an insurer needs before it will process a claim: procedure codes and dates of service per line, the practitioner’s registration number, and the policy and pre-authorisation references — with nothing about the patient leaving your browser.
A private medical invoice has two audiences: the patient, who needs to understand it, and the insurer, who needs to process it. The insurer wins, because an invoice they cannot process comes back — and it comes back needing a procedure code, a date of service, the practitioner’s registration number and the policy or authorisation reference.
This builder carries all four. Each line takes a procedure or service code and its own date, the header takes the practitioner’s name and registration, and the insurer, policy number and pre-authorisation reference sit alongside the patient’s details.
Medical services are exempt from VAT in most jurisdictions when supplied for a therapeutic purpose, so the tax rate starts at zero; cosmetic and non-therapeutic work often is not, and per-line rates handle an invoice that mixes the two. Patient data never leaves your browser, which for this category is not a marketing line.
Not just on the invoice — insurers reject claims without them.
Name, qualification and registration number in the header.
Policy, membership and pre-authorisation codes.
Therapeutic care is usually exempt; cosmetic work often is not.
The whole document is built in your browser.

Policy number, membership number and pre-authorisation code.
Procedure or service code and the date of service on each line, not just on the invoice.

Nothing about the patient is uploaded — the whole document is built in your browser.
The patient’s name and policy or membership number, the pre-authorisation reference, the date of each service, a procedure or service code per line, the practitioner’s name and registration number, and the provider’s details. Missing any one of them is the usual reason a claim is returned.
Medical care supplied by a registered health professional for a therapeutic purpose is exempt in the UK and much of the EU. Cosmetic and non-therapeutic services generally are not, so an invoice can legitimately mix exempt and standard-rated lines.
It never leaves your device. The invoice is built and rendered in your browser and the draft is kept in your own browser storage. There is no upload anywhere in the tool and no server that could receive one.
It depends on the arrangement. Where the insurer settles directly, invoice them with the patient named and the policy referenced. Where the patient pays and reclaims, invoice the patient with everything the insurer will need already on the document.
